A successful transition depends on a well-designed plan and coordinating supports for the individual during adjustment. Students assist in planning and mediating transitions, using assessments and tools, for children with ASD within and across home, school and community settings. Topics include models for effective transition planning assessment and evaluation, consideration for varying stakeholder perspectives and collaboration to develop relationships with parents and professionals.
