ACADEMIC LIFE INSURANCE
Active Academic
Basic Life Insurance and Accidental Death and Dismemberment
Basic Life Insurance is intended to assist with the additional burden of financial hardship in the event of an employee’s untimely death. The amount of Basic Life Insurance is $25,000. In addition to the Basic Life Insurance, employees are covered for Accidental Death and Dismemberment in the amount of $25,000 which is equal to the amount of your Basic Life Insurance.
Supplemental Life Insurance
An employee may choose additional coverage in units of $10,000 to a maximum of 6 units or $60,000 under the Supplementary Life Insurance.
Employee Pay-All Optional Life Insurance
In addition to the Supplementary Life Insurance, an employee may choose additional coverage in Units of $10,000 to a Maximum of 14 Units or $140,000. Available only after maximum Supplemental Life coverage has been elected.
Dependent Optional Life Insurance
Dependent Life Insurance is intended to assist with the additional financial burden should an employee’s spouse/partner or dependent children die. The amount of insurance is $5,000 for a Spouse and $2,000 for each child.
Academic Early Retirees (Age 50-65) “Grandfathered Group”
If you retired prior to September 1, 2005 The Academic Early Retirement Benefits are grand parented and remain in effect. Academic employees could elect to maintain Life Insurance they had up to retirement into retirement up to the age of 65. The retiree pays 100% of all premiums.
Basic Life Insurance
You may have elected to continue coverage in the amount of $25,000 & Accidental Death & Dismemberment of $25,000 equal to your Basic Life Insurance.
Supplemental Life Insurance
Available only if you continued Basic Life Insurance, Equal or less than your pre-retirement amount. Coverage may not be increased once you early retire. Units of $10,000 Maximum of 6 units ($60,000).
Employee Pay-All Optional Life Insurance
Available only if you continued Basic and Supplemental Life Insurance-Equal or less than your pre-retirement amount. Coverage may not be increased once you early retire. Units of $10,000 Maximum of 14 Units ($140,000).
Dependent Optional Life Insurance
Available only if you had family EHC coverage and Dependent Life Coverage. Spouse/$5,000, each Child/$2,000.

ADMINISTRATIVE LIFE INSURANCE
Active Administrative
Basic Life Insurance and Accidental Death and Dismemberment
Basic Life Insurance is intended to assist with the additional burden of financial hardship in the event of an employee’s untimely death. The amount of Basic Life Insurance is 66 2/3% of basic annual earnings rounded to the next higher $1,000 to a maximum of $250,000. In addition to the Basic Life Insurance, employees are covered for Accidental Death and Dismemberment in the amount equal to the amount of your Basic Life Insurance.
Supplemental Life Insurance
An employee may choose additional coverage in units of 1, 2 or 3 times basic annual earnings rounded to the next higher $1,000 to a Maximum of $500,000.
Employee Pay-All Optional Life Insurance
This insurance is available only if the maximum Supplemental Life coverage has been elected. Units of $10,000 to a Maximum of 10 Units or ($100,000).
Dependent Optional Life Insurance
Dependent Optional Life Insurance is intended to assist with the additional financial burden should an employee’s spouse/partner or dependent children die. The amount of insurance is $10,000 for a Spouse and $5,000 for each Child.

Available only if you continued Basic Life Insurance, equal or less than your pre-retirement amount. Coverage may not be increased when you retire. Units of $10,000 to a Maximum of 5 units ($50,000).
Dependent Optional Life Insurance
Available only if you had family EHC benefits and Dependent Life Coverage. The amount of insurance is $10,000 for a Spouse and $2,500 for each Child.
Supplementary Spousal Life Insurance
Is available only if Optional Dependent Life Insurance is continued. Units of $10,000 to a Maximum of 5 units ($50,000).
New Retiree Benefits-Retiree Life Insurance For All Groups
If you retired after September 1, 2005, you are enrolled in the New Retiree Benefit Plan, Number 22182. Academic, Administrative and Support are now under one plan.
All employees can elect to maintain Basic Life Insurance of $10,000.00. Please note that Academic Life Insurance terminates at age 75. All other retirees can maintain their Basic Life Insurance for their lifetime.

Critical Illness Insurance
Available to Academic and Administrative Employees only. Following the diagnosis of a critical illness and a prescribed survival period, this insurance pays a one-time, lump-sum benefit. You are eligible to apply for coverage provided you are an employee of the Colleges of Applied Arts and Technology and you must:
Coverage is available for any amount between the Minimum of $50,000 and the maximum of $250,000, and can be purchased in units of $25,000. Please refer to the Critical Illness Brochure and Information Package available from Human Resources Services.
Making a Life Insurance Claim
Your College Benefits Administrator must be contacted immediately, and will assist you or your beneficiary with the process associated with filing a death claim.
Your Death Claim
In the event of your death, the following must occur as soon as possible afterwards:

An Election of Method of Settlement and Statement of Claim must be completed by the College and your beneficiary.

The completed Statement of Claim along with a death certificate from your attending physician or funeral home must be forwarded to Sun Life in order for the claim to be adjudicated.
Death Claim for Your Dependent
In the event of the death of a dependent, the following must occur as soon as possible afterwards:

An Election of Method of Settlement and Statement of Claim must be completed by the College and you.

The completed Statement of Claim along with a death certificate from your Dependent’s attending physician or funeral home must be forwarded to Sun Life in order for the claim to be adjudicated.
It normally takes 3-4 weeks to process a claim; however, this may vary depending on the circumstances of each case.
