A note on Confidentiality
Self-Identification
It is the student’s responsibility to ‘self-identify’- to contact the CSD as a student with a disability requiring accommodations. Although it is not mandatory to do so, if a student wants to receive services and accommodations, they must register with the CSD.
Who can register with the CSD
CSD services are available to all Algonquin College students: full-time, part-time, apprenticeship, continuing education and distance education. Program applicants who are required to write an Admissions Test can receive accommodations to write the test. Any applicant or prospective applicant is welcome to contact the CSD for information on our services.
To register with the CSD, follow the steps below:
- Collect the documentation required:
You must provide the CSD with specific documents concerning your disability. Here is a list of the documents required. If you are having difficulty getting the documentation (i.e. an out-of-province school) contact the CSD for assistance.

Give the Documents to the CSD
Send, fax or drop off the documents to CSD. Your documents will be kept in a pending file (for up to one year) until you contact the CSD for an appointment. If your scheduled appointment is shortly after you receive your documents, you can bring them to your appointment.
Contact the CSD to make an appointment with a Counsellor
Once you have your documents, or know when you will receive them, contact the CSD to make an appointment with a Counsellor. The first appointment will be approximately one hour long.
4. Come to the appointment with your timetable and any questions you may have. When you come to your appointment, bring your timetable (if you have received it). If you are a continuing education student, bring your Student Timetable which verifies that you have registered.
The Counsellor will review your documentation. Based on the documentation, the program or course you are in, and on your specific learning needs, the Counsellor will type up an Individual Student Plan (ISP). The ISP is the official means of notifying your professor(s) and your program department of the accommodations you are entitled to. The ISP does not disclose what the disability is.
Distribute the Copies of Your Individual Student Plan
The Counsellor will give you copies of your ISP, one for each professor. The original will be sent to the program Chair, Dean or Program Coordinator, and a copy placed in your CSD file.

Attention Program Applicants writing Admissions Tests
If you have been invited to the College to write an Admissions Test for a program you have applied for, you can receive accommodations to write the test. Follow steps 1 and 2 above and then contact the CSD to arrange a test date after the documentation has been received in the CSD.
Attention Distance Education and Continuing Education
For students registered in Distance Education and Continuing Education courses, and who may not be readily available during the CSD office hours, a Counsellor can be available over the phone or through email. Contact the CSD for more information.
A note on Confidentiality
The Centre for Students with Disabilities deals directly with the student. Therefore, information about students registered with the CSD is not given out to third parties unless the student has signed a release form authorizing the CSD to do so.
