IMPORTANT - PLEASE READ CAREFULLY!

 

An Entry Immunization Form is required for some programs.  Screening for immunity to certain diseases and verifying that your immunizations are up to date, is required to ensure that both you and your clients are protected against illness.   To find your correct form visit the form directory.

   
 

Special note to BSCN students ONLY: All immunization records must be submitted directly to the Ottawa University Risk Management Office.  Information on requirements may be found at:

  RETURNING STUDENT?
  Updated TB testing and proof of completion of your Hepatitis B vaccines is required for most programs. If you have had a previously positive TB test, annual TB testing is NOT required.  Please see a Health Services RN to review and update your records.  Once requirements have been updated, you will be given a Returning Student Card to give to your Program Coordinator
   
  PLAN AHEAD!
 
  • Forms must be submitted and completed prior to work placements.
  • It is recommended that you submit your forms early – either before the start date of your program or within the first month.
  • The process usually takes a minimum of 2 weeks to complete, but may take up to 4 weeks -depending on your immunization history. 
 

PLEASE NOTE:

 
    • Some programs have specific deadlines for completion of the forms.  For example, first year BScN students must submit forms no later than August 1st or, in the case of late acceptance, within 10 weeks of the date on the acceptance letter. Information regarding deadline dates, if applicable, are usually included in you acceptance package.  Late forms may result in fines or de-registration from your program.  If you have any questions regarding requirements for your program or deadlines for submitting forms, please contact your program coordinator.

COMPLETING YOUR FORMS:

 

Completion of these forms does not require the services of a physician.  If your doctor fills them out for you, you may be charged a fee.  If your form is completed by your physician, please ensure that he/she signs the document.

The Registered Nurses at the Woodroffe and Pembroke Campuses are available to assist you with your forms, free of charge, no appointment required. Please bring your forms, copies of immunization records and your health card to Health Services.   All required vaccinations, blood work and TB tests are available at the Woodroffe Campus.   TB testing is available at the Pembroke Campus.  TB testing is provided free of charge.  Hepatitis B vaccines are available at a reduced rate for our students - $30 per dose.

   
 

SUBMITTING YOUR FORMS:

 

Copies of your immunization records and any required blood test results (If done at your doctor's office) must be submitted with your forms.

   
  Computerized records of childhood immunizations can usually be obtained by contacting your local Public Health Unit.  If you are unable to locate your immunization records, please speak with an Algonquin College Health Services Nurse.
 

 

  Forms should be submitted to the appropriate campus.  It is recommended that you contact your academic program office to verify that your forms have been received and are complete. (See the appropriate program coordinator links above)
