SUBMITTING YOUR FORMS |
| |
|
| |
FALL INTAKE:
Forms sent by mail and received prior to August 1 will be processed and your program office will be notified automatically if they are complete. If you are missing any information, you will be advised by mail. A letter will be sent to the address you have provided on the form.
DO NOT MAIL FORMS AFTER AUGUST 1st.. If we have not received your forms by August 1, you will need to present yourself to our clinic in room C 141 at Woodroffe Campus to complete processing of your forms. We know that some students are not in Ottawa over the summer, and may have to come in after starting classes. Be aware of any deadlines for your program. It is your responsibility to ensure their timely submission!
After you have submitted your forms, it is recommended that you check with your academic program office to verify they are complete. WINTER or SPRING INTAKE:
Please bring your forms, in person, to Room C141. Distance education students may continue to mail forms but should contact our office to ensure that they are processed.
If you are submitting your forms in person at the Woodroffe Campus, please allow yourself sufficient time to review these forms with the nurse. Due to high volumes during the fall and winter intake period, your patience will be appreciated!
