Clinical Requirements

The mandatory vaccines and health form below are to be completed before clinical placements. Please start this vaccination process as soon as possible as this process must be completed prior to being able to participate in clinical placements.

Screening for immunity to certain diseases and verifying that your immunizations are up to date is a requirement of this program. The process for immunizations can take 2-4 weeks or even longer in some cases, so plan ahead and START EARLY!

2. Algonquin College Health Services

It all starts with a phone call to Health Services to get you started. Registration with our medical clinic is required for services. Please call the campus where you are attending your program of study:

Due to COVID-19, we can only accept booked appointments (no walk-ins) to ensure the safety of all visitors and staff.

The Registered Nurses in Health Services can help you with your forms, free of charge. Please bring your forms, copies of immunization records and your health card to Health Services when you come for your booked appointment.

Completing your forms does not require the services of a physician. If your doctor fills them out for you, you may be charged a fee. If your form is completed by your physician, please ensure that he/she signs the document.

At Health Services:

  • TB testing is provided free of charge
  • Hepatitis B vaccines are available at a reduced rate for Algonquin students
  • Free influenza vaccinations
  • Covid-19 vaccine is not yet available at our clinic
  • If you require blood work, we can provide you with the necessary requisition form to bring to a lab. Blood work can also be done at the Ottawa campus.
  • Some programs have specific deadlines for form completion. If you have any questions regarding requirements or deadlines, please contact your Program Coordinator.

3. Police Records Check Documentation:

Successful completion of clinical placement is a requirement for graduation from the Personal Support Worker program. Agencies that provide placement opportunities require you to have a clear Police Records Check for Service with the Vulnerable Sector (PRCSVS). Your acceptance for placement is at the discretion of the agency. If you register in the program without a clear PRCSVS and as a result are unable to participate in placement, you will not be eligible to graduate.

Clinical Placement Eligibility:

To be eligible for placement, you must submit proof of a clear PRCSVS, which will be retained on your departmental file and used only for purposes related to your placement. You will be required to disclose the contents of the PRCSVS, including all notations, to the placement agencies.

It is your responsibility to obtain the PRCSVS from your local Police Department prior to the deadline identified by your Department and to pay any associated costs. It may take a long time to obtain this documentation; submit your application as early as possible. Should you require further information, contact the Program Coordinator or Academic Chair.

Prior to clinical experience you are required to submit a completed Health Assessment Form and/or an Immunization Form, in order to comply with the legislation and regulations of the institutions which offer field placements or clinical experiences.

Clinical placements receive large numbers of health science students and there may be times during your program of study that require you to attend weekend and evening clinical experiences.
