Before Your First Visit
We can’t wait to welcome you to our Dental Clinic! Below are a few steps you’ll need to complete in preparation for your first visit, including forms to review for medical and personal permissions.
Client Agreement
The below document outlines what you are agreeing to as a client of the Algonquin College Dental Clinic. Review the applicable agreement in advance and plan to take some time at the beginning of your first appointment to complete the form on site.
- Client Agreement Form – Adult (Age 16+)
- Client Agreement Form – Child (Under 16), to be completed by a parent or guardian
X-Ray Radiographs
We require all current x-ray radiographs to be shared with us. You can request copies from your dentist of record to be forwarded to us by filling out our Release Form. Fill this out in advance and bring it with you to your first appointment. In order to protect client confidentiality, please do not email your completed form.
If it’s been more than two years since you last had x-rays, we’ll be happy to take those required to provide care.
List of Medications
Bring a list of any medications you are currently taking. In most cases, your regular pharmacy will be able to print a list of the current medications you have been prescribed.
