Research Application Form for REB Approval

For researchers applying to conduct research at AC. Please note this is the same form accepted at most other Ontario Colleges. It is the researcher’s responsibility to ensure they use the correct form at other institutions. 

If you are planning on conducting research at several Ontario Colleges, we advise referring to the Expert Panel/Multisite Application Process.

Indigenous Community Engagement Form

To be completed and included with application if research is likely to affect the welfare of an Indigenous community, or communities, to which prospective participants belong.

Change of Protocol Form

For researchers seeking approval for any change of protocol to an approved project.

Annual Research Renewal Form

For research projects that extend beyond one year, annual renewal of their ethical approval is necessary.

Project Completion Form

For all researchers to submit once all data collection is complete.
Adverse Event/Unanticipated Issue Report Form

For mandatory reporting of any adverse event or unanticipated issue.
