Thank you for your application to the Food and Nutrition Management Graduate Certificate program.

Our evaluation of your application reveals that the food operations requirement for admission to our program has not been met. However, since you have completed credentials that make you a good candidate for this program we are pleased to accept you into the program under the following condition:

You will take two additional courses in food operations. The courses are FOD2132 Culinary Fundamentals and HOS2236 Food and Beverage Management. These courses must be successfully completed to be eligible to graduate. You are responsible for all costs associated with these additional courses. You will be registered in these courses when space is available.

If you are in agreement with these conditions, please fill out the form below. Your application will be placed on hold until we receive the completed form. Please submit a Program Agreement only if you have been instructed to by the Registrar’s Office via email.
