Additional Forms

The following specific Aviation Management Program requirements must be completed before being assigned to a Flight Training Unit (FTU) (Flight School).

All of the following forms must be completed and emailed to Program Coordinator Greg Mapp, mappg@algonquincollege.com before the start of the first term:

  1. A completed Category 1 Aviation Medical Exam before the start of the first term. Students must receive clearance from a Transport Canada CAT 1 Medical Exam practitioner.
    Important: This process may take as long as 8+ months. Accepted applicants are encouraged to begin the process as early as possible.
  2. Signed Release of Liability form
  3. Signed Authorization for Release of Student Information form.
  4. Review the student handbook and sign all required forms.
    The student handbook guides students and operational personnel conducting flight training at the Flight Training Service Providers of the Algonquin College
    General Arts and Science – Aviation Management Program.
  5. Complete the Additional Flight Information Form

Please email all completed forms to mappg@algonquincollege.com before the start of the first term.

Note: All efforts are taken to assign students to the Flight Training Unit (FTU) of their preferred choice. However, should availability not permit, students are still responsible for their own transportation to the FTU to which they have been assigned – including the Cornwall Aviation FTU – located 90 minutes from the Ottawa area.
