Prior to clinical experience, students are required to fill in this clinical application form and submit the following documents to Paramed:

• Proof of current Ontario Heart and Stroke Foundation Basic Rescuer "C" Level Training.
• A clear Police Records Check for Service with the Vulnerable Sector.
• The Algonquin College Health Assessment Form and/or Immunization Form at the website.

Before you start the application, please ensure that you have a copy of your CNO registration as you will be required to attach it as part of the application process.

Please list your top three clinical location preferences starting with your top preference. ***There is no guarantee that we will be able to accommodate your request.
