Nursing Credential Verification

This information page is for those seeking completion of nursing credentials verification forms. These forms are often related to seeking nursing employment outside of Canada and are a manner for the College or University to confirm completion of a program, course, and/or hours. Examples of these agencies include, but is not limited to, CGFNS International, National Nursing Assessment Service (NNAS), the State of California Department of Consumer Affairs (DCA), and the College of Nurses of Ontario (CNO).

Please note that requests to complete verification forms for programs or courses completed prior to 2012 cannot be fully processed unless course outlines are provided by the applicant. If course outlines are not available for review, details required in the form may be missing.

How to Pay

  1. Call the Registrar’s Office at 613-727-0002 or toll-free 1-800-565-4723.
  2. Make a $100 credit card payment per Algonquin College Program you wish evaluated. (Please note for BScN taken through the University of Ottawa that the fee must be paid to them and the receipt provided to Algonquin)
  3. The customer service representative will put a note on your student file once payment is received

Please review the form before you complete it to ensure you have all the required documentation. Incomplete information will delay processing of your application.
