FALL 2026 COURSE REGISTRATION REQUEST

This form is used to request registration in the courses listed below for the Fall 2026 term for students that are not following the standard progression of their program (e.g., returning from a break in studies or retaking courses from previous levels). Please complete the form carefully and accurately to request registration in your outstanding courses.

Course registration requests are subject to availability, and the submission of this form does not guarantee enrollment in the selected course(s).

After submitting the form, monitor your schedule regularly through ACSIS for any updates or changes. It is your responsibility to review and ensure the accuracy of your academic progression. This includes checking your ACSIS and Brightspace accounts to confirm your timetable, account balance, and academic transcript.

It is important that you complete this form with care. Errors may result in incorrect course registrations or course loads, which could affect your full-time status, co-op eligibility, financial aid or funding, and, for international students, immigration status.
